Unemployment Insurance   

Unemployment Insurance is a program funded by employers. The program provides short-term benefit payments to workers who have lost their job or had their hours reduced through no fault of their own. To learn more about unemployment, visit File for Unemployment – Overview (English only).

Employment Development Department Appeal Form (DE 1000M)
The DE 1000M can be used by claimants and employers to appeal an eligibility decision made by the Department.

Notice of Unemployment Insurance Award (DE 429Z)
Provides claimants with specific monetary and eligibility information on their claim.

Notice of Determination and/or Ruling (DE 1080CZ)
Mailed to employers and/or claimants. Contains information on a claimant’s eligibility for benefits.

Notice of Determination (DE 1080MON)
Mailed to claimants to notify them of their ineligibility for unemployment insurance benefits and information on their appeal rights.

Notice of Unemployment Insurance Claim Filed (Claimant) (DE 1101CLMT)
Provides claimants with a copy of their claim information as processed by the Department. This notice also advises claimants of their right to correct errors and/or omissions, and instructions on how to contact the Department.

Unemployment Insurance Application (DE 1101I)
The DE 1101I can only be printed and completed in English or Spanish, and either faxed or mailed to EDD to file for unemployment benefits. You can find the application in English or Spanish by visiting Unemployment Insurance – Forms and Publications (English only).

Unemployment Insurance Application for Disaster Unemployment Assistance (DUA) (DE 1101ID)
This application includes the supplemental form for Disaster Unemployment Assistance (DUA) if you are unemployed or partially unemployed due to a disaster. The DE 1101ID can only be printed and completed in English or Spanish, and either faxed or mailed to EDD to file for unemployment benefits. Notice sent to employers requesting information about back pay.

Request for Identity Verification (DE 1326C)
Notification advising the claimant that the Department is unable to verify his or her identity and identifying documents must be provided.

List of documents that are acceptable for identity verification.

Notice of Potential Overpayment (DE 1447)
Mailed to claimants. Contains information on a potential overpayment where a claimant may have been overpaid unemployment benefits.

Notice of Potential Overpayment and False Statement Penalty (DE 1447CO-AZ)
Mailed to claimants. Includes information on a potential overpayment and false statement penalties where a claimant may have been overpaid for unemployment benefits.

Response to Potential Overpayment and False Statement Penalty (DE 1447CO-BZ)
Claimants can use this form to provide corrected or additional information about their potential overpayment or false statement penalty.

Notice of Denial of Benefits and Overpayment (DE 1480Z)
Notifies claimants that they were overpaid for unemployment benefits. The notice lists the time period(s) for which the claimants were not eligible for benefits. It also includes information on how to file an appeal.

Notification of Unemployment Insurance Benefits Eligibility Interview (DE 4800)
A notice sent to claimants containing the date and time of their telephone interview regarding their eligibility for unemployment insurance benefits.

Notice of Conditional Payment Pending Eligibility Review (DE 5400E)

Returned EDD Debit Card (DE 5613)
Notifies claimants that their debit card was returned for confidential destruction.

EDD Customer Account Number Notification (DE 5614)

Notice of Appeal and Transmittal of Appealed Determination (DE 6315)
Advises claimants that their former employer appealed the Department’s decision to find them eligible for unemployment benefits.

Notice of Right to Receive Unemployment Insurance Benefits Pending Appeal (DE 6315CC)
Advises claimants that their appeal has been forwarded to the Office of Appeals and explains the right to be paid benefits pending the outcome of the appeal.

Notice of Requirement to Register for Work (DE 8405)
Advises unemployment insurance claimants that they must register for work using CalJOBS within 21 days.

Notice of Determination for Disaster Unemployment Assistance (DE 8598DUA-D)
Notifies claimants that they are not eligible for Disaster Unemployment Assistance.

Get help with filing a claim by phone, general unemployment questions, and technical help with online registration, password resets, and EDD Account Numbers.

UI Automated Self-Service Line    

Get information on how to file a new claim or reopen an existing claim and your last payment issued. You can also certify for benefits using EDD Tele-CertSM, request copies of your 1099G tax information, and find your local America’s Job Center of California.

Disability Insurance

Disability Insurance (DI) is a program funded by deductions from employees’ wages. The program provides benefit payments to workers who have a loss of wages when they are unable to work due to a non-work-related illness, injury, or pregnancy.   2. Select Request Claim Update in the Claim Information section.
  3. Choose a Request Type from the dropdown menu and select Next.
  4. Add your specific question to your claim.

Paid Family Leave

Paid Family Leave (PFL) provides benefit payments to people who need to take time off work to care for a seriously ill family member, bond with a new child, or participate in a qualifying event because of a family member’s military deployment. To learn more, visit Paid Family Leave (English only).

Benefit Overpayments

A benefit overpayment is when you collect unemployment, disability, or PFL benefits you are not eligible to receive. It is important to repay overpayments to avoid collection and legal action. Create a myEDD account to access Benefit Overpayment Services a convenient and secure way to manage your overpayment online.
