- Trade Adjustment Assistance (TAA) and Workforce Innovation and Opportunity Act (WIOA) Co-enrollment Technical Assistance Guide – DE 8306: For WIOA and EDD staff: Use this guide to co-enroll participants in the TAA and WIOA programs.
- Trade Adjustment Assistance (TAA) and Workforce Innovation and Opportunity Act (WIOA) (Co-enrollment Referral Form) – DE 8308: For WIOA and EDD staff: Use this form as a guide to co-enroll participants in the TAA and WIOA programs.
- Application for Trade Adjustment Assistance (TAA) – DE 8309: This is an application to file for the TAA program.
- Alternative Trade Adjustment Assistance (ATAA) Individual Application – DE 8330: This is an application for ATAA. To apply, you must be at least 50 years old, find re-employment within 26-weeks of your original separation date, and earn less than $50,000 per year full time.
- Reemployment Trade Adjustment Assistance (RTAA) Individual Application – DE 8330A: This application is for workers certified under petition series 80,000 and above.
- Reemployment Trade Adjustment Assistance (RTAA) Individual Application – DE 8330 RTAA: This application is for workers certified under the 70,000 series petitions.
- Reemployment Services and Eligibility Assessment (RESEA) Questionnaire – DE 8531: This form is to be completed prior to the RESEA appointment regarding work history and expectations.
- Pre-Screening Notice and Certification Request for the Work Opportunity Tax Credit (WOTC) – IRS Form 8850: This form is used by employers to certify for the WOTC. It must be submitted with either one of the following two forms:
- Individual Characteristics Form – ETA Form 9061
This questionnaire will help to identify a new employee as one of the eligible groups to qualify an employer to receive WOTC. - Conditional Certification – Work Opportunity Tax Credit – ETA Form 9062 This form is filled out by a state agency or partner
