Step 4: Review Benefit Documents

Carefully read and quickly respond to requests for more information.

After you submit your Disability application and required documents, we will mail important documents to you in about two weeks. To avoid any confusion, read the documents we mail to you very carefully. Let us know right away if any information is wrong, such as wages, dates, or missing employers.

  • Notice of Computation (DE 429D)
    This form includes your potential weekly benefit amount, but does not mean you are eligible for DI benefits. immediately to correct any errors to avoid payment delays.
    Note: There is a seven-day unpaid waiting period before benefits start. Your payment starts on the eighth day.
  • Electronic Benefit Payment Notification (DE 2500E)
    If you are eligible for benefits, you will receive this form. It includes important information about your benefit payments.
  • Request for Identity Verification (DE 1326C)
    If you applied by or mail (or we can’t immediately verify your identity), you’ll receive this form. Visit  to learn how to avoid further delays.
  • Disability Claim Continuing Eligibility Certification (DE 2593)
    Sent after 5 payments if you get automatic payments. Fill out and return this form to confirm your disability. If you don’t return it, your benefits will stop.
  • Claim for Continued Disability Benefits (DE 2500A)
    Sent every two weeks or up until the estimated return to work date if you don’t get automatic payments. Complete and return this form to confirm your disability. If you don’t return it, your benefits will stop.
    Note: The quickest way to submit the DE 2593 or DE 2500A is through myEDD if you have an account. These forms must be returned to us within 20 days of the date on the document.

Read all of this mail carefully and let us know right away if any information is wrong. This will help you avoid payment delays.

If you are not eligible, you will receive a Notice of Determination (DE 2517) and an Appeal Form (DE 1000A) with instructions about how to appeal this decision.
