I. DISQUALIFICATION

A. Failure to Request Leave

You quit your last job with (employer name). You did not request a leave of absence to protect your job. After considering available information, the Department finds that you do not meet the legal requirements for payment of benefits.

B. Claim Filed During Leave: Reason for Requesting Leave Not Compelling

You left your last job with (employer name) on a leave of absence for personal reasons. After considering available information, the Department finds that you do not meet the legal requirements for payment of benefits.

C. Failure to Return at End of Leave

You quit your last job with (employer name) when you failed to return from a leave of absence. After considering available information, the Department finds that you do not meet the legal requirements for payment of benefits.

D. Understanding That You Would be Rehired

You left your last job with (employer name) for personal reasons, with the understanding that you would be rehired if work were available. After considering available information, the Department finds that you do not meet the legal requirements for payment of benefits.

E. Leave Offered, Refused

You left your last job with (employer name) but refused a leave that would have held your job for you. After considering available information, the Department finds that you do not meet the legal requirements for payment of benefits.

F. No Longer in Use

II. ELIGIBLE

AA. Failure to Request Leave

The claimant quit your employment without requesting a leave because he/she had a good-faith doubt that he/she would be able to return at the end of the leave. Available information shows that the claimant had good cause for leaving work and failing to request a leave.

BB. No Extension of Leave Available

The claimant left your employment on a formal leave of absence. At the end of the leave, the claimant was unable to return and no extension of the leave was possible. Available information shows that the claimant had good cause for leaving work.

CC. Disciplinary Suspension

The claimant left your employment on a disciplinary suspension and failed to return at the end of the suspension. Available information shows that the claimant had good cause for failing to return.

DD. No Longer in Use

EE. Claim Filed During Leave

The claimant left your employment on an approved leave of absence. Available information shows the claimant had good cause for leaving work.
