Carefully read and quickly respond to requests for more information.
After you submit your unemployment application, we will mail important documents to you in about two weeks. To avoid any confusion, read the documents we mail to you very carefully. Let us know right away if any information is wrong, such as wages, dates, or missing employers.
- Notice of Unemployment Insurance Claim Filed (DE 1101CLMT)
This is a summary of your claim information. It explains how to correct any errors to avoid payment delays. - Employment Development Department Customer Account Number Notification (DE 5614)
This has your EDD Customer Account Number (EDDCAN) to register in UI Online, after you visit myEDD. The EDDCAN also helps verify your account if you speak with an EDD representative. Don’t lose it. - Notice of Unemployment Insurance Award (DE 429Z)
This form explains what benefits you can expect, and how to avoid payment delays. If any information is wrong, follow the instructions and appeal in writing within 30 days to avoid payment delays. - Unemployment Insurance Benefits: What You Need to Know (DE 1275B)
This provides you a link to a document that explains how you can avoid payment delays by accurately certifying for benefits every two weeks. - Continued Claim (DE 4581)
You can use this form to certify by paper, but the process is much slower. Save time and do it online at myEDD or by phone. You’ll certify for benefits every two weeks. - Request for Identity Verification (DE 1326C)
If you applied by phone, fax, or mail (or we can’t immediately verify your identity), you’ll receive this form. Visit Respond to Your Request for Identity Verification to learn how to avoid further delays.
