I. DISQUALIFICATION
A. Death of Relative or Friend - Definite Period of Time

D. Occupied With Personal Affairs - Indefinite Period
You are/were occupied with personal activities. After considering available information, the Department finds that you do not meet the legal requirements for payment of benefits.
E. Out of Area - Definite Period of Time
You were out of town on personal business from (date) to (date). After considering available information, the Department finds that you do not meet the legal requirements for payment of benefits.
F. Incarcerated More Than Two Workdays
You could not work because you were incarcerated or detained from ______________ to _________________.
G. Incarcerated - Indefinite Period
You cannot work because you are incarcerated.
H. Work Alternative in Lieu of Incarceration
You could not work because you were in a work alternative program in lieu of incarceration from _______________ to _______________.
I. Incarcerated Not More Than Two Workdays - Court Disposition Not Final
You could not work because you were incarcerated or detained from ______________ to ________________. After considering available information, the Department finds that you do not meet the legal requirements for payment of benefits. If the charge filed against you is dismissed, you must notify the Department promptly. A reconsideration of this decision cannot be done unless the Department is notified within 15 days of the dismissal.
J. Not Available for Full-Time Employment
You are not willing to accept full-time employment because you are holding a public office. After considering available information, the Department finds that you do not meet the legal requirements for payment of benefits.
K. Willing to Work - Limited Labor Market Due to Voluntary Restriction
You have placed limits on your work because you have retired from your usual occupation. After considering available information, the Department finds that you do not meet the legal requirements for payment of benefits.
L. Pension/Social Security Precludes Other Work
You have placed limits on your work because you will not accept employment that affects the receipt of your pension or Social Security benefits. After considering available information, the Department finds that you do not meet the legal requirements for payment of benefits.
M. Self-Employed During Customary Working Hours
You are self-employed during the normal working hours for your occupation. After considering available information, the Department finds that you do not meet the legal requirements for payment of benefits.
N. Unavailable During Specific Period
You could not work from _______ to _______ because you were involved in self-employment activities.
O. Restricts to Self-Employment - No Elective Coverage
You will only work in self-employment. After considering available information, the Department finds that you do not meet the legal requirements for payment of benefits.
P. Restricting to Part-Time or Intermittent Work
You are only willing to accept part-time or intermittent work because you are self-employed. After considering available information, the Department finds that you do not meet the legal requirements for payment of benefits.
Q. Conflict With Customary Working Hours - Family Enterprise
You are involved in a family business during the normal working hours for your occupation. After considering available information, the Department finds that you do not meet the legal requirements for payment of benefits.
R. Preoccupation With Self-Employment Activities
You have placed limits on your work because you are engaged in activities preliminary to entering self-employment. After considering available information, the Department finds that you do not meet the legal requirements for payment of benefits.
S. Does Not Want to Work
You have made a personal choice not to work. After considering available information, the Department finds you do not meet the legal requirements for payment of benefits.
II. ELIGIBLE
AA. Death of Relative or Friend
You state the claimant may not be available for work because he/she has been involved in activities connected with the death of a relative or friend. After considering available information, the Department finds the claimant is able and available for work.
BB. Occupied With Personal Activities
You state the claimant may not be available for work because of involvement with personal activities. After considering available information, the Department finds the claimant is able and available for work.
CC. Incarcerated Less Than One-Half Day
You state the claimant may not be available for work because he/she was incarcerated. After considering available information, the Department finds the claimant is able and available for work.
DD. Incarcerated Not More Than Two Working Days
You state the claimant may not be available for work because he/she was incarcerated from ____________________ to _________________. After considering available information, the Department finds the claimant was incarcerated for two days or less and is able and available for work under Section 1253.1 of the Unemployment Insurance Code.
EE. Serving as Juror or Court Witness
You state the claimant may not be available for work because he/she is serving as a juror or as a court witness. Under the provisions of Section 1253.6 of the Unemployment Insurance Code, the claimant is exempt from availability for work requirements during periods of such service.
FF. Willing to Accept Full-Time Work - Public Official
You state the claimant may not be available for work because of his/her duties as a public official. After considering available information, the Department finds the claimant is able and available for work.
GG. Receipt of Old Age and Survivor’s Disability Insurance

HH. Willing to Work Despite Effect on Receipt of Social Security Benefits/Pension
