I. DISQUALIFICATION

A. Domestic Reasons - Failure to Explore Alternatives

You quit your last job with (employer name) because of domestic reasons. You did not explore all reasonable solutions before you quit. After considering available information, the Department finds that you do not meet the legal requirements for payment of benefits.

B. Duties, Children - Failure to Explore Alternatives

You quit your last job with (employer name) to stay home with your children. You did not explore all reasonable solutions before you quit. After considering available information, the Department finds that you do not meet the legal requirements for payment of benefits.

C. Child Care Problems - Did Not Explore Alternatives

You quit your last job with (employer name) because of child care problems. You did not explore all reasonable solutions before you quit. After considering available information, the Department finds that you do not meet the legal requirements for payment of benefits.

D. Relocation After Marriage - Could Have Commuted

You quit your last job with (employer name) when you were married and moved. You could still have commuted to work. After considering available information, the Department finds that you do not meet the legal requirements for payment of benefits.

E. Relocation - Failure to Request Transfer

You quit your last job with (employer name) because you were moving. You did not request a transfer. After considering available information, the Department finds that you do not meet the legal requirements for payment of benefits.

F. Family Illness - Presence Not Required

You quit your last job with (employer name) because of illness in your family. Others were providing actual care. After considering available information, the Department finds that you do not meet the legal requirements for payment of benefits.

G. Family Illness - Could Have Had Leave of Absence

You quit your last job with (employer name) to take care of a relative. You did not request a leave of absence to protect your job. After considering available information, the Department finds that you do not meet the legal requirements for payment of benefits.

H. Quit to be Married - Could Have Continued Working

You quit your last job with (employer name) to be married. You could have continued working after your marriage. After considering available information, the Department finds that you do not meet the legal requirements for payment of benefits.

I. Quit to Prepare for Marriage - Marriage Not Imminent

You quit your last job with (employer name) to prepare for your marriage. Your marriage was to take place some time in the future. After considering available information, the Department finds that you do not meet the legal requirements for payment of benefits.

J. Quit to Accompany or Join Nonmarital Partner - No Children

You quit your last job with (employer name) to join your nonmarital partner in another area. After considering available information, the Department finds that you do not meet the legal requirements for payment of benefits.

K. Quit for Honeymoon

You quit your last job with (employer name) to go on a honeymoon. After considering available information, the Department finds that you do not meet the legal requirements for payment of benefits.

L. Did Not Wish to be Away From Family

You quit your last job with (employer name) because you did not want to be away from your family. After considering available information, the Department finds that you do not meet the legal requirements for payment of benefits.

M. Death of a Family Member

You quit your last job with (employer name) because of a family illness/death. You did not explore all reasonable solutions before you quit. After considering available information, the Department finds that you do not meet the legal requirements for payment of benefits.

N. Domestic Violence Abuse - Failure to Explore Alternatives

(Use VQ 155 A)

II. ELIGIBLE

AA. Compelling Domestic Obligations

The claimant quit your employment because of domestic reasons. Available information shows that the claimant had good cause for leaving work.

BB. Moving After Marriage - Outside Normal Commute Area

The claimant quit your employment to move with his/her spouse to a place outside the normal commute area. Available information shows that the claimant had good cause for leaving work.

CC. Moving After Marriage - No Transfer Available

The claimant quit your employment to move with his/her spouse. He/she was unable to transfer to another worksite nearer the new home. Available information shows that the claimant had good cause for leaving work.

DD. Family Illness or Death - No Leave Available

The claimant quit your employment because of a family illness/death. Available information shows that the claimant had good cause for leaving work

EE. Unemancipated Minor

The claimant quit your employment at the insistence of his/her parents. The claimant is a minor, subject to parental control. Available information shows that the claimant had good cause for leaving work.

FF. Domestic Violence Abuse - No Reasonable Alternative
