Notice of Reduced Earnings (Fisherperson)
The DE 2063F is a form used by employers to certify their employees for partial benefits for fisherpersons. Employers can print and complete the Internet version for submission to EDD. Please review the DE 4210F, Completion Instructions for Notice of Reduced Earnings, (Fisherperson), for more information on partial claims for fisherpersons.

Request for Eligibility Information
This form is sent to claimants who did not attend a required Reemployment and Eligibility Assessment appointment. Claimants must complete the four page form (DE 4365REA) and questionnaire (DE 4365REAQ), and mail it to the EDD within 10 days of the mailing date listed on the form. This form may be printed, completed, and mailed to the EDD.

This form is used by a retired annuitant to request UI records for the hiring state agency to ascertain whether he/she received benefits within the previous 12-month period.

Request for Eligibility Information Due to Failure to Attend a Mandatory Appointment
This four-page form is sent to claimants on a first or second tier federal extension claim who did not attend a mandatory Reemployment and Eligibility Assessment (REA) 2012 appointment. Claimants must complete the multi-page questionnaire (DE 4365REAX) and return it to the address on the form within 10 calendar days. Claimants must report to the One-Stop Career Center listed on the original appointment notice as soon as possible to complete their in-person REA appointment and to minimize the potential number of weeks for which they may be disqualified for failing to attend their original appointment.

This form is sent to claimants who have been scheduled to attend a Reemployment And Eligibility Assessment (REA) appointment. Claimants must complete the form prior to the REA appointment and bring the completed form to the REA appointments.
