Use Code Section 1256, issue VQ, and the appropriate letter.

I. DISQUALIFICATION

A. Prevented Employer From Adjusting Grievance

You quit your last job with (employer name) but did not tell your employer why you were quitting, preventing him/her from solving the problem. After considering available information, the Department finds that you do not meet the legal requirements for payment of benefits.

II. ELIGIBLE

AA. Separation is a Layoff, Lack of Work

You contend the claimant quit your employment. After considering available information, the Department finds that the separation was a layoff for lack of work.

BB. Company Shutdown/Temporary Layoff

The company scheduled a vacation shutdown. You state the claimant left your employment on a leave of absence instead of using vacation leave. After considering available information, the Department finds that the separation was due to a lack of work because the claimant did not have the option of working during the shutdown period.
