I. DISQUALIFICATION

A. Union Agreement Violation

You quit your last job with (employer name) because you felt the employer violated the terms of the union agreement. After considering available information, the Department finds that you do not meet the legal requirements for payment of benefits.

B. Employer or Union Discrimination or Intimidation

You quit your last job with (employer name) because of harassment on the job. After considering available information, the Department finds that you do not meet the legal requirements for payment of benefits.

C. Union Disciplinary Action

You quit your last job with (employer name) because of union disciplinary action. After considering available information, the Department finds that you do not meet the legal requirements for payment of benefits.

II. ELIGIBLE

AA. Violation of Collective Bargaining Agreement

The claimant quit your employment because of a violation of the collective bargaining agreement. Available information shows that the claimant had good cause for leaving work.

BB. Union or Employer Discrimination or Intimidation

The claimant quit your employment because he/she was harassed on the job. Available information shows that the claimant had good cause for leaving work.
