Use Section 1256, VQ40, and the appropriate letter.

I. DISQUALIFICATION

A. VQ to Attend School

You quit your last job with (employer name) to attend school. After considering available information, the Department finds that you do not meet the legal requirements for payment of benefits.

B. Leave of Absence to Attend School

You left your last job with (employer name) to go on a leave of absence to attend school. After considering available information, the Department finds that you do not meet the legal requirements for payment of benefits.

C. School Attendance Required - Failure to Resolve Before VQ

You quit your last job with (employer name) to attend required schooling. You did not explore all reasonable alternatives to quitting before you left. After considering available information, the Department finds that you do not meet the legal requirements for payment of benefits.

D. Training, Retraining, or Apprentice Program - No Prior Approval Given

You quit your last job with (employer name) to enter a state-approved training, retraining, or apprenticeship course. When you quit you had not been accepted into the course. After considering available information, the Department finds that you do not meet the legal requirements for payment of benefits.

E. Training (Including GAIN) - No Conflict in Hours Between Training and Work

You quit your last job with (employer name) to attend a training course. You could have attended the training outside your regular work hours. After considering available information, the Department finds that you do not meet the legal requirements for payment of benefits.

F. VQ From Employer-Sponsored, Paid Training

You quit a paid training program with (employer name). After considering available information, the Department finds that you do not meet the legal requirements for payment of benefits.

II. ELIGIBLE

AA. School Attendance Legally Required

The claimant quit your employment to return to school as required by state law. Available information shows that the claimant had good cause for leaving work.

BB. Approved Enrollment in State Training, Retraining, or Apprenticeship Course

The claimant quit your employment to attend a State-approved training, retraining, or apprenticeship course. Available information shows that the claimant had good cause for leaving work.
